In addition to registering as a guest to reserve your spot, New Hope Ministries requires that all guests and parents/caretakers that join us for Naples Dance with the Stars complete a media consent form.

Please download and complete these forms, and submit your completed copy via email to guests@naplesdancewiththestars.com prior to the evening of Dance with the Stars. If you are unable to submit via email, please print and bring the completed form with you the night of the prom.
